As a Navian Hawaii hospice Facility Manager, you will guide patients and their family on their journey, delivering comfort and support with a caring heart, replacing fear with hope, helping them find the beauty and joy in every moment, and providing each person with the kindness and dignity they deserve.
Primary
Responsibilities:
Leadership Facilitates communications and works effectively with home staff, patients/families/caregivers, physicians, vendors, and other departments and professionals as appropriate.
Communicates agency information to facility staff.
Provides leadership, problem solving, and management decision-making and demonstrates a commitment to quality and corporate accountability.
Consistently represents the facility to all internal and external customers with integrity and professionalism and provides for a safe environment for patients, guests, visitors and employees.
Demonstrates good judgment in seeking and accepting guidance, facilitates clear and honest communication with other team members.
Demonstrates ethical conduct and judgment that is in harmony with the mission and values of Navian Hawaii.
Demonstrates ability to execute an effective, supportive, sensitive approach in response to the needs of staff and the patient and family.
Compliance Assures implementation of and compliance with all policies and procedures, licensing and certification rules, regulations and guidelines (Chapter 94 and CMS).
Responsible for managing compliance with licensing and Medicare certification, including all rules and regulations from Chapter 94 and CMS, as applicable.
Facilitates patient care and quality management activities by assuring staff participation and home representation on focus groups; follows up on surveys and grievances; performs medical record and utilization review audits.
Adheres to all policies, procedures and regulations regarding patient care, conduct, safety, fire, security and risk management.
Schedules and oversees regular and intermittent safety maintenance of the facility and equipment.
Make recommendations to the DCO for necessary structural or equipment upgrades and replacements.
Patient Care Case manager for all inpatients at the Navian Hawaii facility Oversees delivery of patient services in Navian Hawaii Home.
Assures adequate and appropriate staffing and scheduling for the provision of patient services 24/7 and is available for 24-hour/ seven-day back up on call.
Monitors documentation and implementation of physical, psychosocial, and spiritual interventions for the patient/family/caregiver.
Supervises all facility staff and provides for ongoing education.
Responsible for the performance of the staff by orienting new staff, collaborating with Director of Clinical Administration regarding disciplinary action, completes staff annual evaluations, competencies (i.
e.
Safety, CPR, etc.
), and maintains documentation of current licenses, CPR, First Aid, TB clearance, and PE.
Expected to participate in the Performance Improvement Program and to comply with Federal and State regulations governing hospice programs, and to adhere to the home's standards of conduct as a condition of continued employment.
Financial Responsible for financial management for the home including budget preparation, expense approvals, contracting issues as appropriate, and being responsive to all financial issues.
Approves all time sheets, schedules, and time off requests.
Performs other duties as required.

Experience and Educational Requirements:
Registered Nurse (RN).
BSN preferred with a current license in good standing to practice as a registered nurse in the State of Hawaii.
Nursing Home Administrator (NHA) license strongly preferred.
Minimum of 2 years healthcare supervisory experience required preferably in a Hospice Inpatient or Residential Unit, a Skilled Nursing Facility or an Expanded Care Home.
Expected to participate in appropriate continuing education as may be requested and/or required by Director of Clinical Administration.
In addition, expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities, and to attend mandatory educational programs.
Working Conditions:
Primarily indoors in a 24/7 home setting with occasional duties outdoors in all types of weather.
Hours:
Generally Monday through Friday, 8:
00 am through 4:
30 pm.
However, position requires being available on call after hours including weekends and holidays, as needed.
